How to Request Assistance

If you need mortgage assistance, the first step is to call 800-441-3456 to speak with a Payment Assistance Specialist. We’ll review your financial situation and discuss the best options for you.

To process your request, you’ll need to provide the following documents:

  • Completed Assistance Application Form (signed and dated)
  • Completed 4506-C Form (signed, dated, and attestation boxes checked)
  • Your two most recent pay stubs (or four if paid weekly)
  • Your two most recent bank statements (showing name, address, and transaction details)
  • Written explanation for any bank deposits over $250 (excluding payroll)
  • Most recent federal tax return (signed and dated) or last year’s W-2
  • Documentation of all other household income
  • Any supporting documents related to your financial hardship
  • If your home is listed for sale, a copy of the listing agreement
